An individual or organization can serve as both a Trusted Contact and a Guardian. Once you add them, Pawmize will send a digital consent form by email from care@pawmize.com, by text from (866) 604-5758, or both, depending on your preference.
The form includes your pet’s medical history, vaccination records, and behavioral information so they can understand your pet’s needs before agreeing to the role. They must sign the form before Pawmize can contact them to help put your Care Plan into action if needed.
Each year, we’ll send an updated consent form asking them to reaffirm their willingness to serve.
